for Resident-Centered Care
(MARRCC)
Care Plan
The MARRCC Care Plan Section was designed to assist
recreational professionals working in Long Term Care (LTC)
to consistently write care plan goals that are measurable,
related to the problem, and resident-centered. The care plan
library includes sample goals and interventions/approaches for
each functioning level of each domain.
Purpose
- Assist the recreation professional in developing care plan
goals that are measurable, directly related to the problem, and
based on the resident's assessed needs.
- Provide the recreation professional ideas to choose from
when developing care plan goals as well as provide guidelines
to assist residents in developing their own goals.
- Assist the recreational professional in determining the
need to revise the residents' goals and interventions.
When to Complete
An individualized care plan should be developed for each
resident upon admission to the facility. The care plan should
be reviewed on a quarterly basis in coordination with the
quarterly MDS and care plan conference meeting. In addition to
this, it should be reviewed, and revised if necessary, upon the
resident's readmission to the facility, once per year in
conjunction with the resident's annual MDS, and any time the
resident demonstrates a major change in health or ability.
Time Needed to Complete
Varies depending upon the resident's individualized needs and
abilities.
