for Resident-Centered Care
(MARRCC)
Annual,
C.O.C., and Re-Admit Assessment
The MARRCC-Annual/C.O.C./Re-admit was designed to assist
recreational professionals working in Long Term Care (LTC)
in assessing and evaluating the effectiveness and
appropriateness of the individualized care plan developed
for the resident. Where the MDS is utilized, this assessment
should be completed prior to the annual MDS, and should
constitute the information source for the Activity section of
the MDS.
Purpose
- Assist the recreation professional in evaluating the
effectiveness and appropriateness of the resident's
individualized care plan.
- Enable the recreation professional to monitor change in
the resident's functioning level and individual needs within
each domain.
- Assist the recreational professional in determining the
need to revise the residents' goals and interventions.
- Enable the recreation professional to track and monitor
change in residents' functioning levels and individual needs,
and adjust the recreation program designed to meet those needs.
When to Administer
The MARRCC-Annual/C.O.C./Re-admit should be administered upon
the resident's readmission to the facility, once per year prior
to the resident's annual MDS, and within 14 days of a major
change in health or ability.
Time Needed to Administer
Approximately 15-30 minutes.
